Medical Forms:

• Medical Information Form – required for ALL students
• Anaphylaxis Emergency Action Plan– required for students who have life threatening allergies and carry and EpiPen
• Form B – required for students with Diabetes
• Form C – required for students who have Asthma and carry a puffer
• Form D – required for students who have seizures
• Form E – required for students with any other serious/life threatening medical concern
• Form F – required for students who need, (or may need) assistance to take medication.
